1. Start your LPA form online at the Office of the Public Guardian (OPG) website. You may appoint 1 or 2 donees to act on your behalf.
2. Wait for donees to accept online. Your donees will be notified of their role via email or SMS.

What is the cost of an LPA?
There are two main costs when making an LPA: certification by a certificate issuer (such as a doctor, lawyer or psychiatrist) and the application submitted to the Office of the Public Guardian (OPG).

If you require customised powers beyond the standard provisions in Form 1, an LPA Form 2 must be drafted by a qualified lawyer. According to Ministry of Social and Family Development data retrieved on 6 July 2025, the median fee charged by lawyers to make an LPA is around $100.
From 1 April 2026, the OPG application fee for LPA Form 1 has been waived (i.e. free) for all Singapore citizens. For non-citizens, the application fee is $90 for permanent residents and $230 for foreigners.
