Lasting Power of Attorney (Co-organised with DBS Bank)

  1. Campaign
    • 1.1 This Complimentary LPA Certification Campaign (the “Campaign”) is organised by DBS Bank Ltd (“DBS”) in partnership with Parkway Shenton Pte Ltd (“Parkway Shenton”), an LPA certificate issuer (“CI”).
    • 1.2 Under this Campaign, eligible customers will be entitled to a complimentary LPA certification with Parkway Shenton, subject to these Terms and Conditions.
  2. Campaign Period
    • 2.1 The Campaign is valid from 1 January 2026 to 31 March 2026 (both dates inclusive), or such other period as may be determined by DBS at its discretion.
    • 2.2 DBS may withdraw, suspend or end the Campaign at any time without giving prior notice or reason.
  3. Eligibility Criteria
    • 3.1 To qualify for the complimentary LPA certification, a customer must satisfy all the following criteria:
      • (a) A Singaporean Citizen.
      • (b) Completed a financial planning review, known as Your Financial Journey (“YFJ”), with a DBS Wealth Planning Manager (“WPM”).
      • (c) Your Qualifying Product must be signed between 1 January 2026 and 24 March 2026 (both dates inclusive) (as defined in Clause 4), and issued by 27 March 2026 at the time of booking the LPA appointment.
    • 3.2 Each eligible customer is entitled to one (1) complimentary LPA certification under this Campaign, regardless of the number of Qualifying Insurance Products purchased.
    • 3.3 The Campaign is not transferable and is strictly for the benefit of the eligible policy owner only.
  4. Booking an Appointment with Parkway Shenton
    • 4.1 Eligible customers may book an appointment with Parkway Shenton via the following:
      • (a) By phone via the Parkway Shenton Hotline at +65 6812 6782 and quote “Parkway Shenton DBS LPA” for appointment making.
      • (b) By email to smg.customerservice@parkwayshenton.com.sg, providing Full Name, NRIC number, Date of Birth, Gender, Contact Details and first & second preferred appointment dates.
    • 4.2 To ensure that their LPA administration can be completed at the clinic during the appointment, patient will need to complete these:
      • (a) Access the OPGO portal (Office of the Public Guardian Online).
      • (b) Complete the online LPA application form, including the appointment of their donee(s). From now until 31 March 2026, OPG is waiving its application fee for one (1) donee, subject to OPG’s terms.
      • (c) Ensure that their appointed donee(s) have logged in to OPGO and accepted the appointment.
  5. List of Parkway Shenton Participating Clinics
    • 5.1 The list of Parkway Shenton clinics are as follows:

      Please note that the list of clinics is subject to the availability of LPA accredited doctors anchored at these clinics.

  6. LPA Redemption at the Clinic
    • (a) Present the eNote received from DBS.
    • (b) Provide the voucher number stated in the eNote, together with your NRIC, to Parkway Shenton for verification and registration.

Terms & Conditions

  1. General
    • 1.1 DBS may at any time, without prior notice or liability, revise, delete, suspend, withdraw and/or add to any of these Terms and Conditions, or terminate the Campaign.
    • 1.2 DBS shall not be responsible for any loss, claim, liability, expenses or damages suffered or incurred by any person in connection with this Campaign, including but not limited to any act or omission by Parkway Shenton, OPG or any third party.
    • 1.3 By participating in the Campaign and/or booking the complimentary LPA certification, the customer is deemed to have read, understood and agreed to be bound by these Terms and Conditions and any other terms that may be imposed by DBS, Parkway Shenton and/or OPG.
    • 1.4 In the event of any dispute, the decision of DBS shall be final and binding.
    • 1.5 These Terms and Conditions are governed by the laws of Singapore and the parties submit to the exclusive jurisdiction of the courts of Singapore.
  2. Cancellation or Rescheduling of Appointment
    • 2.1 Customers who wish to cancel or reschedule their LPA certification appointment must contact Parkway Shenton using the channels in Clause 4.
    • 2.2 Any conditions, administrative fees or consequences of late cancellation or no-show will be subject to Parkway Shenton’s prevailing policies, which may be amended from time to time.
  3. Qualifying Insurance Products
    • 3.1 “Qualifying Insurance Products” refer to all of the following, that are distributed by DBS and issued by Manulife (Singapore) Pte. Ltd. during the Campaign Period:
      • (a) All Regular Premium Participating (“PAR”) policies.
      • (b) All Regular Premium Investment-Linked Policies (“ILP”).
      • (c) All Protection Plan with min 500 APE paid annually.
      • (d) All Single Premium Participating (“PAR”) policies, excluding “Manulife Goal” policies.
    • 3.2 General insurance products and any other products not specified above will not qualify for this Campaign unless otherwise notified by DBS in writing.
    • 3.3 All Qualifying Insurance Products must be purchased via the advisory channel through a DBS Wealth Planning Manager.
