  1. Items described in packages are meant only for the patient's treatment of the package condition only and are not transferable.

  2. Only standard medications are covered in the packages.The number and type of standard medication dispensed will be entirely at the doctor’s discretion.

  3. Non standard medications for treatment of condition not stated in the packages are not covered. Patients have to pay for these medications but will be given a discounted price by the clinic.

  4. Treatment of other medical conditions other than the specified illness will be charged separately outside these packages. Patients will be given preferential pricing at the discretion of the physician.

  5. The patient will follow up at the same clinic for his/her treatment. This is in line with MOH's recommendation that every chronic illness patient should have his/her own personal family physician so that better outcomes of care can be achieved.

  6. Medisave processing is only applicable for the Chronic Disease Management packages only.

  7. Refund policy - there will be no refund after 3 months .Before 3 months, if patient insists on a refund, we will calculate the refundable portion after deducting the adminstration fee and the services rendered at the clinic.
